Documents and Forms

 

 

You will not be allowed to practice until all these forms are filled out and complete on Family ID. 

 

ATTENTION - FOR THE 2018 SEASON, MOST OF THE REGISTRATION FORMS ARE TO BE COMPLETED ON A WEBSITE CALLED Family ID. 
The registration portal can be accessed HERE.

The Tracy Unified School District Athletic Handbook (revised) can be viewed HERE this is not to be confused with the Team Handbook/Agreement which is to be reviewed, signed and returned to the coaches.

 1.Required forms are now on Family ID and no longer need to be filled out (Physical Exam/Health Insurance, Pre history medical, Head Injury).

 

2. Special Use Forms

Transportation forms are required if  you are going to be a driver of athletes other than just your child to and from swim meets:

• Use of Private Vehicle to Transport Students 

• Permission Form for Students to Ride With Other Drivers

 

The team agreement is posted to the left. Read and sign the form, and bring it with you the first day of practice.
